You can register with our dental practice using the registration form on the right. We will contact you by telephone as soon as possible to schedule your first appointment.

For your first appointment, we ask you to arrange a few things in advance so that we can make your visit as smooth as possible.

• ​Please send us your dental records and X-rays from your previous dentist. This will give us a complete picture of your oral health. You can send these to info@tpzuyderhoven.nl.

• ​Please bring the following documents with you: proof of identity (with your national insurance number) and insurance details.

• ​We ask that you arrive 10 minutes early for your first appointment.

• We will use this extra time to fill out a medical questionnaire so that we can fully understand your health situation before we begin.

During this intake appointment, we will take the time to get to know you and your teeth. We will examine the condition of your teeth and gums, discuss your wishes and take X-rays for a complete and thorough initial assessment. This will enable us to provide you with the right advice and guidance straight away.

After the first visit, we usually schedule an appointment with the prevention assistant or dental hygienist, followed by any follow-up treatments with the dentist.
