Practice terms and conditions
Below you will find the terms and conditions and information regarding our practice, appointments and rates.
Registration

You can register with our practice using the online registration form on the website or at the reception desk at our practice.

We ask for accurate and up-to-date personal details on the registration form. Before your first appointment at our practice, we also ask for your national insurance number and valid ID.

By registering, you agree to our practice terms and conditions and privacy statement.
Deregistration

You can deregister at any time by notifying us by email.

We will process your deregistration within 5 working days.

You have the right to have your patient file transferred by us. We will do this directly to you, so that you can then transfer it to your new healthcare provider.

Any outstanding invoices must be paid before your file can be transferred.
Treatment

Prior to treatment, we will inform you about the treatment, the risks and the costs.

For extensive or costly treatments starting at €250, a written estimate will be provided.

Treatments are carried out when the patient gives their consent (verbally or in writing).
Rates

We apply the national NZa rates for oral care. These can be found at www.tandarts.nl.
Payment

We work with Payt Software, which means you will receive the invoice from our practice name.

Invoices must be paid within the specified period.

In the event of late payment, the invoice may be transferred to a collection agency. Any additional costs will be borne by the patient.

If your invoice has been transferred to a collection agency, all treatments during the collection process must be paid for directly at the practice by debit card or cash.
The first appointment (intake)
Before your first appointment, we ask you to arrange a few things in advance so that we can make your visit as smooth as possible.

You can register with our dental practice using the registration form on the website. We will contact you by phone as soon as possible to schedule your first appointment.

Please send us your dental records and X-rays from your previous dentist. This will give us an immediate overview of your oral health. You can send these to info@tpzuyderhoven.nl.

Please bring the following documents with you: proof of identity (with social security number) and insurance details.

We ask you to arrive 10 minutes early for your first appointment.

We will use this extra time to fill out a medical questionnaire so that we have a good understanding of your health situation before we begin.
Making a follow-up appointment

Follow-up appointments are made immediately at the reception desk.

If this is not possible, you can make a follow-up appointment by telephone.
Canceling or rescheduling
Cancellations or rescheduling must be made by telephone at least 48 hours in advance. Only working days are counted in this period.
If you cancel too late or do not show up without notice, you may be charged for the reserved time. These costs are not covered by your health insurer.
Reminder
You will receive a reminder by email both 7 days and 3 days before the appointment. This ensures that you have enough time to reschedule the appointment if it is not convenient. If you do not have email, you will receive the reminder by text message.
Reminders by email and text message are sent by us as a service. In the unlikely event that you do not receive them, the appointment made will remain valid.
We believe it is important to continuously improve our care. That is why we would like to hear about your experience visiting our practice. Suggestions, comments, and positive experiences are all valuable to us. This helps us identify what is going well and where we can make adjustments.
Do you have any questions, concerns, or are you dissatisfied with something? We warmly invite you to discuss this with us in person. Often, we can quickly find a suitable solution together during a conversation. If you prefer, you can also send us your experience or complaint by email.
We take every report seriously and are happy to talk to you to find a solution together.
In the unlikely event that we are unable to reach a solution, you can use the KNMT complaints procedure. This procedure allows you to engage an independent complaints officer free of charge. They will help you and us to find a solution together.
If this process also fails to produce a solution, you can turn to the Oral Care Disputes Board, where your complaint will be assessed independently.

Please note the following:

- When making an appointment by telephone, please have the social security number of the person for whom the appointment is being made at hand. Please also provide the name of our dental practice.

- You must bring valid proof of identity with you.
-
- The emergency dentist does not have access to your file. They will focus on providing emergency care and you will be referred back to our own practice for any follow-up treatment.

- A limited list of dental care rates related to emergency oral care will be used. Legally established surcharges may apply for evening, night, weekend, and holiday services.

- After treatment, you will be referred back to us.

- You (and we as your own dentist) will receive an invoice, discharge letter, and any photos taken.

We are legally obliged to keep a patient file containing relevant medical data.

In accordance with the WGBO (Medical Treatment Agreement Act), we retain your medical data for 15 years from the date of the last change to your file, unless a longer retention period is necessary.

You have the right to access, correct and transfer your file.

We treat your privacy with care and process your data in accordance with the GDPR.

These terms and conditions apply to all services we offer as a dental practice. By using our healthcare services, you agree to these terms and conditions. We strive to provide high-quality, safe and transparent care. Where these terms and conditions refer to “patient”, this also includes the patient's legal representative.
Changes to terms and conditions
Tandartspraktijkl Zuyderhoven reserves the right to amend these terms and conditions. The most recent version can always be found on our website.
The practice is located on the border between Amstelveen and Aalsmeer and is easily accessible from both places.

The practice has ample parking on its own premises.

Bicycle
The location is next to the Zuyderhoven neighborhood and is easily accessible by bicycle. There is a bicycle parking facility on site.

Public transport
The nearest bus stop is approximately 400 meters from the practice.
Thanks to its central location, the practice is easily accessible by both private and public transport.
